The name and address of:
A. the person selling the goods to the purchaser, or
B. the person consigning the goods to Canada.

The date the goods began their continuous journey to Canada, usually date of pickup. Your customs broker will use this information to calculate the exchange rate. If you do not know the exact date an estimate will suffice.
Used to record additional information (eg. the commercial invoice number, purchase order number) that could aid in the reconciliation of documents.
The name and address of the person or company in Canada to whom the goods are being shipped.
The name and address of the person or company to whom the goods were sold.

The country through which goods are transported en route to their final destination but do not actually enter that country's economy.
The country where the goods were grown, produced, or manufactured. If there is more than one country of origin, please list with each line item in field 12.
The mode of transportation and the place from which the goods began their uninterrupted journey to Canada.
The terms and conditions agreed upon by the vendor and the purchaser (eg. Net 30 Days, FOB).
The currency in which the value on the invoice is calculated by (eg. USD, CDN, EUR).
The total number of packages in the shipment.
The following information must be provided:
- kind of packages
- general description of goods (eg. women's skirts, automotive springs)
The quantity of each item included in the description fields, including the appropriate unit of measure.
The value in the currency of settlement (as defined in field 10) for each item listed in the description field.
The price paid or payable for the number of items recorded in the description field. If the item is a sample or has no commercial price, it still needs to list a value that reflects fairly the worth of the item for customs purposes.
Both the net and gross weight in lbs are required.
- net weight: the weight of the goods being shipped or purchased.
- gross weight: the net weight plus the weight of the container, trailer, shipment or packaging.
The total amount owing or the total price paid or payable for goods described on the invoice and on continuation sheets (if necessary).
List any identifying numbers for the shipment (eg. Purchase Order, Sales Order, Invoice Number).
The name and address of the person or company shipping the goods to the consignee, if different than the vendor.

If the Canada Customs Invoice (CCI) or the Commercial Invoice (CI) is completed by someone other than the consignee or the vendor, indicate the name and address of the completing agent. This field may be left blank if the information is provided elsewhere on the invoice.
List the number and date of any applicable Canada customs rulings to the shipment.
